From 1st July 2025, Medicare is implementing changes to Chronic Disease Management. Here are the changes and how it affects you as a patient.

 

Use the table as a quick understanding of the major changes from 1st July 2025.

 

To register with MyMedicare, you can do it directly through your Medicare account by nominating Queens Park Medical Centre and your preferred GP. They will be the only doctor able to complete your GPCCMP until you deregister with us.

 

Alternatively, you could visit us at the centre or on your next visit, ask one of our friendly receptionist for the physical form to complete. Once completed, hand it back to reception and we will register on your behalf.

FAQs

1. What happens to my current care plan?

Nothing changes! The government is allowing the care plans to be in place till 1st July 2027. So two years of being on your current plan. Once the care plan is “finished” and new assessment is required, you will automatically be switched to the new system.

2. Do I need to do anything?

Absolutely nothing. The best thing you can do for yourself and your GP, is to register with MyMedicare and nominate your GP. Without this, it could mean that you could be registered to another GP or not at all!

3. Who is eligible for MyMedicare registration?

Patients who can register with MyMedicare have to hold a valid Medicare card.

Fee Increase from 1st July 2025
From 1st July 2025, there will be an increase to our practice fees and treatment room fees.

 

Saturday Private Billing
We are no longer bulk billing on Saturdays. Patients with a valid concession card or OSHC holders will be expected to pay a minimum of $40 out of pocket gap fee on Saturdays.
